For the reporter to UNDERSTAND and ASSESS the risk associated with the incident – A risk assessment using the following table is ALWAYS required.

Baby Scans 4d expect all incident reports to be completed in full and forwarded to the compliance email – pending the RISK outcome score the following steps should be taken.

• LOW RISK – < 6 = Report to compliance
• MEDIUM RISK – < 14 = Report to compliance RM Informed verbally Documented in communication folder Consider immediate controls
• HIGH RISK – > 15 = Report to compliance RM Informed immediately verbally Documented in communication folder Consider external reporting notifications (Police , CQC, Safeguarding, Riddor)
